Supporting Documents Required (Do not send originals, each photocopy must be certified*)
Proof of Identity - Each applicant must provide one document from each of the 4 categories (4 documents needed):
1.
Proof of right to be in Australia - Birth Certificate or Citizenship Certificate
2.
Proof of photo and signature - Drivers Licence or Passport or NSW Photo Card issued by the RTA
3.
Proof you live in Australia - Medicare Card or Motor Vehicle Registration or Centrelink Card
4.
Proof of Address - Utitility documents (ie. Gas, Water, Electricity Bills) or Insurance Policy
Proof of Name Change and/or Marital Status - Provide a certified copy of the below if you are:
Married - your marriage certificate
Divorced - your divorce certificate
Widowed - death certificate of your spouse
Separated - a statutory declaration
Transaction Proof - Provide information for one of the 3 types of transactions:
Contract to Purchase a new or established home - Copy of exchanged Contract for Sale, dated and signed by the Vendor and Title Search showing the applicant
Contract to Build a home - Contract to Build dated and signed and Title Search and Occupation Certificate issued by your Local Council or a Final Inspection Certificate
Owner Builder - Approval from Council for the laying of the foundations and the Occupation Certificate issued by Council or a Final Inspection Certificate. Documentary proof of building costs and a Title Search
* Certified Copies - A copy of an original that has been sighted by an authorised person and who has noted on the copy that is a true copy of the original which has been produced and shown to the person certifying. An authorised person includes: A member of the police force, a councilor of a municipality, a medical practitioner, a dentist, a veterinary practitioner, a pharmacist, a minister of religion, a legal practitioner, a school principal or an accountant.
